Job Overview
We are seeking a dedicated and highly skilled Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. The ideal candidate will possess a comprehensive understanding of primary care, acute and chronic disease management, and a passion for delivering exceptional patient-centered care. As an FNP, you will provide comprehensive health assessments, develop treatment plans, and collaborate with physicians and specialists to ensure optimal health outcomes across diverse patient populations, including pediatric, adult, geriatric, and special needs individuals. Responsibilities

  • Conduct thorough patient assessments including vital signs, physical examinations, and medical history reviews to establish accurate diagnoses.
  • Develop and implement personalized treatment plans encompassing medication administration, injections, IV infusions, wound suturing, and other procedures as necessary.
  • Manage patient care across various settings such as outpatient clinics, urgent care centers, nursing homes, assisted living facilities, and home care environments.
  • Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks to document clinical encounters accurately and efficiently.
  • Perform diagnostic evaluations including laboratory specimen collection/processing, sonography, and basic medical imaging when appropriate.
  • Provide health coaching and education tailored to individual patient needs, including chronic disease management like diabetes or hypertension.
  • Collaborate with multidisciplinary teams on discharge planning, case management, utilization review, and telehealth services to ensure continuity of care.
  • Maintain compliance with HIPAA regulations and ensure all medical documentation adheres to legal standards.


Requirements

  • Valid Family Nurse Practitioner license with certification from the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
  • Proven experience in primary care with additional expertise in areas such as gastroenterology, geriatrics, pediatrics, behavioral health, or acute pain management preferred.
  • Proficiency in EMR/EHR systems including Epic, Cerner, or Athenahealth for documentation and patient data management.
  • Strong clinical skills including suturing, phlebotomy, catheterization, sterile processing techniques, and IV insertion.
  • Experience working with diverse populations such as children, seniors in memory or dementia care settings, or individuals with developmental disabilities.
  • Knowledge of CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), and utilization review processes.
  • Ability to perform complex procedures like spinal taps or cardiac catheterization when necessary; hospital experience in ICU or Level I Trauma Center settings is advantageous.
  • Excellent communication skills with the ability to educate patients effectively about their health conditions and treatment options.

Shift:
Must be willing to float between Gainesville and Macclenny as needed. The schedule includes 2–3 weekday shifts and 6 out of every 12 weekend shifts (Saturday/Sunday), totaling approximately 30–38 hours per week. This position may also require participation in holiday rotation coverage.
